How I Can Support You

Support Coordination is about helping you understand your NDIS plan, connect with appropriate supports and build a support network that works for you.

My role is to help make the process clearer, coordinate the different parts of your support team and reduce some of the stress that can come with navigating the NDIS.

Understanding Your Plan

I can help you better understand:

  • Your NDIS funding

  • What different funding categories can be used for

  • Your plan goals

  • Plan management arrangements

  • How your supports fit together

  • Areas where funding may be underused

  • Gaps in your current supports

The aim is to help you feel more confident and informed about how your plan works.

Finding the Right Supports

Finding the right providers can take time.

I can help research and connect you with services that suit your individual needs, preferences and goals.

This may include:

  • Allied health professionals

  • Support workers

  • Behaviour Support providers

  • Community and social supports

  • Employment and education supports

  • Plan managers

  • Disability and community services

Where possible, I aim to provide choice and support you to make informed decisions about the providers you work with.

Coordinating Your Support Team

When several providers are involved, communication can become complicated.

I can assist with communication between participants, families, nominees and providers so that everyone has a clearer understanding of the participant’s goals, current supports and responsibilities.

This may include:

  • Provider liaison

  • Coordinating communication between services

  • Multidisciplinary meetings

  • Following up outstanding actions

  • Identifying service gaps

  • Supporting smoother transitions between providers

When Things Aren’t Working

Sometimes services stop being the right fit.

I can help identify concerns, communicate with providers, explore alternatives and work with you to find a more suitable way forward.

My role is to help you feel supported while navigating changes and to assist you in maintaining services that are appropriate, safe and aligned with your goals.

Remote and In-Person Support

I work with participants across Australia through remote Support Coordination.

For participants located in Adelaide metro and the Adelaide Hills, in-person support can also be available where appropriate.

This gives participants the flexibility to choose the type of support that works best for them.
